hospital in Rajahmundry. They didn't provide me any invoice copy, after many calls

Hello I need an information, one month back I made a debit card transaction in one of the hospital in Rajahmundry. They didn't provide me any invoice copy, after many calls they provide me a bill receipt. But I had a insurance policy where I can claim the amount, but for that they are asking the invoice copy. I called to the hospital and ask for the same. But the doctor is saying as you made the transaction through card we will not provide you any invoice copy. So can some one tell how to proceed further if I didn't receive invoice copy. Also they charged me 2% extra at the time of payment as I made the transaction through card
